CONFERENCE CONSENT FORM

The consent form is a required form for all conference participants and volunteers. 

The form must be completed and postmarked before May 1st.

Please mail the notarized document to:

If the participant/volunteer will be under the age of 18 at the time of the conference, please note that the form will require the signature of a parent or guardian.  If the parental signature is required, it must be signed in the presence of a Notary.  

Upon review, you will notice that the consent form references MYLead Policies.  MYLead values the safety and security of all of our participants.  For that reason, we like to share these policies with our participants upfront so you know what to expect.
